Health Screening Form

This health screening form will need to be completed for any individual requesting access to campus. Before coming to Adat Ari El, everyone must  answer all of the questions on the form. If you are attending Shabbat morning Tefillah, you must fill out the form within 24 hours of the service start time (For example: If you are attending services at 9:30am on Saturday morning, you can submit your form any time after 9:30am on the Friday before you attend). 

By completing the form you are agreeing to adhere to all safety protocols outlined by Adat Ari El from time to time, and the failure to do so will result in removal from campus.

Be advised of the following:

  • When arriving at Adat Ari El, you will be subject to additional health screening measures including having temperature taken by a staff or security member before being allowed access to campus.
  • Proper masks and social distancing of at least 8 feet while in Tefillah and 6 feet while moving about campus are mandatory at all times while on campus. (Please do not wear a bandana as a mask, you will not be given access without a properly fitted mask covering both nose and mouth)
  • While on campus, you will be expected to comply with all signage and instructions from staff members relating to safety protocols.

Suspected exposure to a person that is sick will potentially exclude an attendee or staff member from accessing campus. If you suspect you are sick or have been exposed, please reschedule and DO NOT come to campus.
